1. View the video segment on Care/Storage/Documentation.
   • This video segment is a supplement to Module 3 content and should assist in strengthening the understanding of the caring and storage of medication once received into the school setting and proper documentation procedures of medication administration.
   • Please note: Although Colorado is referenced throughout the video segments, the content is applicable to the District.
2. Approximate time to complete Module 3 (excluding assignments): 20 minutes
3. Complete Worksheet 3 (Find the Errors) on page 45 and Worksheet 4 (Transcribe the Order) on page 47 in your training booklet.

Complete the following assignment:

1. Please print Worksheet 3 (Find the Errors) on page 45 and Worksheet 4 (Transcribe the Order) on page 47 in your training booklet.
2. Find the errors on the authorization form in Worksheet 3, then transcribe the corrected order onto the medication administration form in Worksheet 4, filling in your answers directly on them.
3. Scan or photograph (using a smart phone) both completed worksheets and then upload them as one attachment below.
